Health Insurance Card

 

It is important for you to carry your health insurance ID card with you at all times because the card is your proof of health coverage when you visit the doctor or hospital to receive medical care. Your health insurance card has the name of your insurer, your name, member ID number, and group number which tells your health care provider the type of plan you have (i.e. what doctors you can see, what benefits you have, how much you have to pay, etc.). If you need prescription drugs, your pharmacy will check your health insurance card to verify what drugs are covered and the cost of the medications. In case you have questions about your coverage, the card lists the contact number for member services and additional information. Be sure to have your health insurance ID card readily available whenever you receive medical care for verification of your health benefits.
