CCHRC is its patients’ partner in disease management. Physicians of the Chinese Community Health Care Association refer their patients to visit CCHRC for dependable bilingual health information. Through an understanding of their disease condition by attending health education seminars, meeting with a dietitian or a health educator, reading health education materials/books, visiting websites, or watching health videos, patients are able to exercise self-care and thereby manage their conditions better.
